Making a Referral to Circles Alternative Education

Please refer to the instructional video on submitting a referral but if you have any questions please give us a call.

• If possible, make yourself a tea or coffee before sitting down to complete the form.

• The form needs to be completed in  one sitting, ideally of no more than 15 minutes. 

If you’re school or agency not currently working with us, we’ll need to complete a service level agreement with you via DocuSign before we can approve the referral and begin provision.
